How to Enroll
The Enrollment Process
- Organizations must complete a simple application. The application can be submitted on-line and paid by credit card, or downloaded and mailed to our office with an enclosed check or valid Purchase Order. Before your completed application can be processed you will be asked to read & agree to the terms and conditions of the ARS membership agreement.
- The term of the ARS Membership is for one annual period. Membership may be renewed for subsequent annual periods by paying the prevailing rate.
- Your organization must designate a contact person to act as the liaison between your organization and CIHQ. This individual will receive time sensitive communications and be responsible for disseminating CIHQ correspondence to throughout your organization.
- ARS Membership is reserved for facility organizations only. Independent consultant and individual enrollment applications will not be considered.
