Why Care Coordination?...

Too often fragmented healthcare results in poor patient outcomes, hospital readmissions, and medication errors. Effective and sustainable care coordination bridges the gap between primary care, acute care, specialty care, and community resources to ensure holistic, patient-centered health services.

In this education we will discuss current care coordination, with the multiple service lines, and the ongoing growth of care coordination. Following the presentation, the participant will better identify aspects of care coordination, specifically Chronic Care Management (CCM), Annual Wellness Visits (AWV), and Behavioral Health Integration (BHI).
